The CU Sports Medicine and Performance Center was formed with staff from Boulder Center for Sports Medicine/Boulder Community Health and CU School of Medicine in August of 2015. All of our physicians, physician assistants, and nurse practitioners are CU SOM employees. All of our physical therapists, imaging staff, physician support staff, outreach athletic trainers and front desk staff are BCH employees. This gives our patients the benefits of both Boulder Community Health and the University of Colorado Medicine with a large depth of resources.

Policies
Our clinic is able to serve you in-person or via virtual health services. Please call our clinics directly or visit MyHealthConnection to schedule an appointment for both in person and virtual health.- When scheduling an appointment, please be prepared to provide the reason, or chief complaint, for your appointment.
- Please provide at least 24-hour notice if you are unable to keep your scheduled appointment.
- Please arrive 15 minutes before your scheduled appointment for check-in.
- Patients arriving more than 10 minutes after their scheduled appointment time may be asked to reschedule their appointment.
- We ask all new and established patients to present their photo identification and insurance card at every office visit.
- Patients should anticipate paying required co-pays at check-in.
- CU Medicine clinics are tobacco-free environments. We do not allow smoking or the use of tobacco products (including e-cigarettes) in our clinics or buildings.
- CU Medicine prohibits the presence and use of knives, guns, or other weapons in our clinics or buildings. This prohibition includes patients, visitors, and staff members with concealed weapons permits.

Parking information
FREE PATIENT PARKING
Please park in lot 390, just north of the Champions Center. If the lower parking lot is full, feel free to use the upper lot.
Please come to the second floor for COVID screening and parking validation. (you can also complete e-check-in on the day of your visit through your My Health Connections account)
All patients and visitors will need to bring their own protective mask,
this cannot be a bandana, neck gaiter/buff, or mask with a ventilator.
